The Life Long Care Program was initiated by the Long Term Care Division of the Ontario Ministry of Health.We provide community-based culturally appropriate services to urban Aboriginal elderly, frail, physically disabled, chronically ill individuals which promotes an integrated response, community participation, independent living and an improved quality of life.


Programs and services are designed to the physical, mental, emotional and spiritual needs of people who require:
