The Life Long Care Program was initiated by the Long Term Care Division of the Ontario Ministry of Health.We provide community-based culturally appropriate services to urban Aboriginal elderly, frail, physically disabled, chronically ill individuals which promotes an integrated response,
community participation, independent living and an improved quality of life.

Determination of eligibility is by the intake process as deemed by the Ministry of Health and Long Term Care Health-Care Program Division.
